When a student exits special education due to graduation from secondary school or due to exceeding the age eligibility for FAPE, the student must be provided a 
summary of his/her academic achievement and functional performance which shall include recommendations on how to assist the student in meeting his/her 
postsecondary goals. [41.305(5)c] Either a Summary for Postsecondary Living, Learning, and Working form, or a Support for Accommodation Request form (SAR) must be completed to meet this requirement.


If going on to a college/university, please consult with the receiving institution as to what form they require or prefer.
